Your medical records contain information about each doctor's visit, lab and radiology report, and other health information. If you need to transfer care from Grand River Medical Group to another office, you can complete a release of information request.

Release of Information Request
If you need to request that your medical records be sent to an outside clinic, you must complete a release form. You can complete your Authorization Form for Release of Information online, in person, by mail, or by fax.

Release of Information for Minors
If you are completing the Authorization to Request Medical Records for your child the parent/legal guardian MUST sign the document and state their relationship to the patient.
